• Employees and each covered dependent they have on the CPG Healthcare plan are eligible for an annual $200 vision benefit.
  • This $200 benefit can cover:
    • Eye exams
    • Eye glasses
    • Contacts
    • Safety glasses
      • This benefit can be combined with the CPG PPE reimbursement of $100 every 2 years towards prescription safety glasses. 
  • To submit your claim, please perform the following:
    • Enter a ticket on the HR Support Ticket system
    • Code it as a Healthcare - Vision Inquiry
    • Scan and attach your vision receipt and the vision invoice if one is available.
    • The CPG Medical Reimbursement Form must be included with your receipt. 
  • How long should I expect to before being reimbursed?
    • Preferred Administrators, CPG's Third Party Administrator processes all vision claims. 
    • Preferred only issues reimbursements and cuts checks once per month.
      • So depending on when your claim is received the processing time could be:
        • Same month as submitted 
        • At the longest, the following month
